The Transnational Forum on Integrated Community Care (TransForm) is a joint initiative of foundations in and beyond Europe that aims to put the community at the centre of primary and integrated care. It is meant to help enable communities to develop their own models of caring by putting people and needs at the centre of planning.
The Graham Boeckh Foundation is a partner in TransForm along with several European foundations including Compagnia di San Paolo, Robert Bosch Stiftung, Fondation de France, King Baudouin Foundation, the Network of European Foundations, and the International Foundation for Integrated Care.
TransForm’s core principles are well aligned with the Integrated Youth Services projects that are being developed in several Canadian provinces with support from GBF. The TransForm initiative offers a unique opportunity to include youth services in the field of integrated community care, which is already a mature concept in the home and elder care sectors. TransForm also allows approaches to be shared globally as well as between Canada and the rest of the world.
